Hunks in Heels 2025
Consent & Application Form

IMPORTANT INFORMATION
Please read and sign Sections 1 & 2, then complete and submit the application in Section 3.   Thank you!

SECTION 1:
Participant
Agreement

Please read everything in this section and complete the signature block to confirm you have read, understand, and agree to all of our rules and conditions. 

Remember, you are the STAR! Let’s make the evening memorable! 

 

Three judges will be scoring on the following categories:

  • Choreography

  • Outfit/Attire

  • Audience Appeal

  • Eye Contact

  • Rhythm

  • Lyric Memorization

  • Facial Expressions

  • Song Engagement

PARTICIPANT INFO: ​

  • Attire: Your choice – have fun!

  • Heels: Your choice – have fun! 

  • Individuals MUST be able to attend a meet & greet/dress rehearsal earlier in the day on Saturday JULY 20, 2024 – please keep that day open!

  • Please email a headshot to SpreadSpencersSparkle@gmail.com to use on social media and the program.

  • A changing room and food will be provided for participants.

  • We can reserve a 8 top table for your “Fan Club.” Each person sitting at your table MUST purchase a ticket to the event. Hunks are the ONLY people who do NOT need a ticket.

SONGS:

Songs must be submitted to SpreadSpencersSparkle@gmail.com with the subject line ‘HUNKS IN HEELS’ by July 1st for approval. 

  • We need two song choices, a performance song AND an introduction song.

  • Please ensure song lyrics fit PG-13 guidelines.

  • Performance songs need to be around 3 minutes long.

  • Performance songs may be a mashup of different songs.

  • When our DJ announces you, that will be your introduction song.

  • Once chosen and submitted, songs cannot be changed.

**For any trims, edits, mashups, etc., please email song with a detailed description of the outcome you are seeking for your music THREE WEEKS before the show.

DURING the PERFORMANCE:

  • Prop mics will be provided (no real microphones).

  • You may wear a headset microphone if you have one.

  • Points will be DEDUCTED for the following actions:

-Removal of clothes

-Walking off the stage

-Throwing objects to the audience 

-Inviting others onto the stage

SIGNATURE

I have read, understand, and agree to the participant, song, and performance rules and conditions, including how the Hunks in Heels 2024 contest will be judged and scored.

SECTION 2:
Release and Waiver of Liability and Indemnity Agreement

Please read this waiver information section and complete the signature block to confirm you have read, understand, and consent to this agreement. 

Participant(s), defined as a person participating in the event knowns as HUNKS IN HEELS, HEREBY RELEASE(S), WAIVE(S), DISCHARGE(S) AND COVENANTS NOT TO SUE ALL OF THE FOLLOWING ENTITIES/PERSONS: Spread Spencer’s Sparkle, Mr. and/or Mrs. Donnie Stephens, whose mailing address is 10192 Willowood Avenue, Englewood, FL 34224 and the Englewood Elks Lodge #2378, located at 401 N. Indiana Avenue, Englewood, FL 34223,  and its owners and any other responsible party, and each of them, their officers and employees, all for the purposes herein referred to as “Releasees”, from all liability  to the undersigned(s), his/her/their personal representatives, assigns, heirs, and next of kin for any and all loss or damage, and any claim or demands therefor on account of injury to Lessee’s person or property or resulting in death or serious bodily injury of the undersigned, whether caused by the negligence of the Releasees or otherwise while the Lessee is in, or upon, the leased premises, and/or, the property, Englewood Elks Lodge #2378, 401 N. Indiana Avenue, Englewood, FL 34223 including its appurtenances (collectively “the premises”) for any purpose.

LESSEE HEREBY ASSUMES FULL RESPONSIBILITY FOR AND RISK OF BODILY INJURY, DEATH OR PROPERTY DAMAGE regardless of whether said injury is due to the negligence of Releasees and/or Lessors or otherwise while in or upon the premises located at 401 N. Indiana Avenue, Englewood, FL 34223 for any purpose and/or reason while present upon the above described premises.

THE UNDERSIGNED expressly acknowledges and agrees that some of the activities described, herein may be very dangerous and involve the risk of serious injury and/or death and/or property damage.

 

THE UNDERSIGNED further expressly agrees that the foregoing release, waiver, and indemnity agreement is intended to be as broad and inclusive as is permitted by the law of the state of Florida which the premises are located and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect.

THE UNDERSIGNED HAS READ AND VOLUNTARILY SIGNS THE RELEASE AND WAIVER OF LIABILITY AND INDEMNITY AGREEMENT, and further agrees that no oral representations, statements, or inducements apart from the foregoing written agreement have been made.
